We collect only what a small dental office needs to answer you and to keep the website working.

- Appointment request details you enter, such as your preferred day, time, and reason for the visit
- Basic usage analytics, if and when the practice enables an analytics service. As published today this site runs no analytics or advertising trackers at all
- Standard server information that any web server records, such as IP address and the time of a request
What we do not collect here
Please do not send detailed health information through this website. The public site is not the right place for medical history, medication lists, diagnoses, images, or anything else about your health.

How we use your information
We use what you submit to respond to you, to schedule or confirm appointments, to answer questions about services and costs, and to keep basic records of our communication with you.
If analytics are enabled, we use them in aggregate to understand which pages people find useful and where the site is confusing, so we can improve it. We do not sell your personal information, and we do not share it with advertisers for their own marketing.
Protected health information and HIPAA
Bellaire Dental Group is a healthcare provider, and the health information created in the course of your care is protected health information under HIPAA. That information is handled under HIPAA and under the practice's Notice of Privacy Practices, which you receive as a patient of the office.
Information you type into a public website form is not automatically protected health information, and it may travel over the internet without the safeguards that apply inside the practice. That is the reason for the request above: keep health detail off the website and give it to us directly instead.
Third-party services
This website relies on a small number of outside services, and information you enter into them is handled under their own terms and privacy policies rather than ours.
Applying for third-party patient financing is a separate process handled entirely by the financing company. The practice does not receive or store your financing application or the financial details you submit to them.

How long we keep information
Contact and appointment request information is kept for as long as needed to respond to you and to run the practice, and then for as long as applicable law requires records to be retained.
Analytics data is retained according to the settings of the analytics service the practice uses. Dental records themselves are retained under the rules that apply to healthcare providers in Texas, which are separate from this website policy.
Your choices and how to reach us
You can ask what website contact information we hold about you, ask us to correct it, or ask us to delete it. You can also ask us to stop contacting you at any time. Requests about your dental records themselves, including access and amendment rights under HIPAA, are handled through the practice directly rather than through this policy.
