Why the paperwork exists
Dental forms are not busywork. They are how Dr. Valter finds out what she needs to know before she treats you.
Medications and health conditions change dental treatment in real ways. Blood thinners change how we plan an extraction. Certain heart conditions and joint replacements change what happens before an appointment. Pregnancy changes what imaging we take. Allergies change which anesthetic and which materials we use.
None of that is visible in your mouth. It only comes from you telling us.
What we ask about
- Your contact details and who to reach in an emergency
- Your medical history, including conditions being treated and past surgeries
- Every medication and supplement you take, including doses if you know them
- Allergies and past reactions, especially to medications, latex, or anesthetic
- Your dental history, past treatment, and anything that has gone badly before
- Dental insurance details, if you carry coverage
- Consent and privacy acknowledgments required for treatment
Getting your forms before the visit
- Book your appointment. Request a time through our online portal, or call or text (713) 668-8383.
- Ask which forms apply. New patients, returning patients, and parents completing forms for a child are not all asked for the same paperwork. Our front desk will tell you which set is yours and how it will be sent to you.
- Fill them out at home. Take your time and check your medication list against the actual bottles rather than from memory.
- Arrive a few minutes early. Even with paperwork done, a short buffer on a first visit keeps everything relaxed.
What to bring with you
- A photo ID
- Your dental insurance card or plan details, if you have coverage
- A written list of your medications and doses
- The name of your previous dental office, if you would like records transferred
- For a child's visit, the parent or legal guardian who can sign consent

Which forms do I need to complete?
Call or text (713) 668-8383 and our front desk will tell you which forms apply to your visit and how to receive them. What is required differs for new patients, returning patients, and children.
Can I fill out the paperwork at the office instead?
Yes, though it takes time out of your appointment. Completing forms beforehand means more of your visit goes to the exam and your questions.
Should I send my health information through the website?
No. Please do not put medical detail into a public website form. Call or text (713) 668-8383 and our team will tell you the appropriate way to send it.
Who fills out forms for a child?
A parent or legal guardian, and that same person needs to be present at the visit to give consent for treatment. We see children starting at age 2.
Can you transfer records from my old dentist?
Bring the name of your previous dental office and mention it to our front desk. They will tell you what is needed to request your records.
